To get the spindle out you don't need to touch the threaded adjuster rings ? 

If the nut on the other side unscrewed it should knock out, but it may be crusted in by corrosion ?

Nut is out on the right hand side ok.  Nothing is shifting when tapped with a hammer.  Not sure if it's threaded on left hand side shown in the picture

The adjusters are threaded in to the frame, the pivot shaft is not threaded. Try soaking it with penetrating oil and letting it sit, then try hitting it with a hammer again to get it out.
