93-95 GSX-R 750 top yoke: offset and fork spacing

Hi - I'm having a machinist fab me a custom top yoke.  It will replace a GSX-R750 piece.  I've searched a bit, but came up empty.  Could someone please tell me the offset (distance from stem hole center to line connecting fork hole centers) and the fork spacing?

Have a 93 gsxr 750 front end here so info is 100%. 

Fork centers = 214mm 

Offset = 35mm

Slingshot 750 U.S.D. are also 214mm fork centers and 35mm offset.
